Referring Veterinarian Form

Client Behavior History Form

Although referral is not required, we welcome the opportunity to work with your primary care veterinarian, neurologist, and/or surgical facility.

We believe in a team-based approach to veterinary behavior and rehabilitation medicine.

Medical euthanasia housecall visits will require medical records from your pet’s regular veterinarian.

Please note that we only provide primary care to animals with documented extreme difficulty receiving primary care in general practices due to behavior or limited mobility.

We will send letters to your animal’s referring veterinarian to keep them in the loop with your animal’s progress. Please send records to housevetstaff@gmail.com or fax them to 734-661-4388.
